Request Food Assistance

North Fork Women is offering Food Assistance to current members of North Fork Women through food gift cards. Cards will be sent during two months, From November 19, 2025 — December 18, 2025; and from December 19, 2025 — January 18,  2026. Applicants apply for one month at a time. 

The current month (November — December) is open for applicants. The December — January month will be available for signup on 12/19/25.

ALL of the following specifications must be met in order to receive food assistance:

• You must be a current member of North Fork Women as of November 16, 2025.
• You must complete the simple application by clicking the button below. If you have any questions write them in the ‘Notes’ section and submit — we’ll get back to you.
• Food cards can be given for any one of four local North Fork supermarkets listed on the form (IGA, King Kullen, Stop & Shop, or ShopRite) or digital cards can be given to other markets for those that live outside of the North Fork area (if available.) The application below will have space for your preferred market.
• Only one food market can be given  per applicant.
• PLEASE NOTE: All information given is strictly confidential. Only one person will read your application, check your eligibility and send your card.
